Welcome to the Rural Community Health Toolkit. This toolkit provides rural communities with the information, resources, and materials they need to develop a community health program in a rural community. Building a rural community health program begins with collecting information about the community’s assets, needs and challenges, and the factors that affect health. This information can be used to develop goals, set priorities, and identify the types of resources that will be needed to create a program. Rural program planners should consider involving affected communities, intended audiences, and key partners in identifying priorities for community health programs.
