Project AWARE & TISS Sustainability Planning Learning Community

CMHIS launched a six-session, national learning community from February-April 2026 to support state, tribal, and district teams to identify local facilitators and barriers to program sustainment and create practical sustainability plans. During the sessions, teams engaged with structured tools and planning templates to support development of their plans.

The learning community was designed to help participants:

  • Develop tailored sustainability plans: Create a roadmap to sustaining the work by identifying sustainment priorities, barriers/facilitators, strategies, and outcomes
  • Learn practical sustainability approaches: Build knowledge of evidence-based strategies and steps involved in sustainability planning that go beyond funding
  • Focus on actionable tools: Apply structured tools, including the Program Sustainability Assessment Tool (PSAT) and planning worksheets, to guide planning and decision-making
  • Connect with coaches and peers: Learn alongside CMHIS implementation coaches and AWARE and TISS grant teams from across the nation through discussion and problem solving

Recordings of the six learning sessions and accompanying materials are available below to support Project AWARE and TISS grantees in their sustainability planning. We encourage users and their teams to partake in the breakout activities shared in each learning session and complete the optional “homework” assigned at the end of each session to get the full learning community experience.
